
New Medical Form

For obvious reasons, it is important for the troop to keep accurate, up-to-date medical and emergency contact information on hand for every scout and scouter.
We also must have current, signed permission forms for each scout to participate in our activities.
Boy Scouts of America has a new form that we will now use. It consists of four parts:
Page 1 – Instructions
Part A – Medical History
Part B – Physical Examination
Part C – Parental Permission
For now, we need a completed Part A and Part C (the medical history and permission portions of this form) for every boy and adult leader.
Please fill out return those portions of the form and return them to Scoutmaster Brad Baier as soon as possible.
You do not need to complete Part B (the physical exam portion) at this time. It requires a medical doctor’s signature and will only be needed for those who will be attending summer camp. We will provide more information about that as the time for camp approaches.
